Disaster Assistance Questionaire
As your church family, it is important to us to have an emergency plan in place during this difficult fire season. The purpose of this questionnaire is to compile information so that we can move forward in preparing a plan. In the event that members of our congregation are required to evacuate in an emergency, we want to provide assistance as needed.
To do that, we need volunteers who are willing to help in one or more of the following areas:
- Provide assistance to those who may need help preparing to evacuate.
- Host evacuees in your home. Note: You will not be asked to house anyone who is not a regular attendee of our church.
- Make phone calls to match up evacuees with host families.
Additionally, we would also like to compile a list of individuals and families who may need assistance in evacuating, so we can know in advance what kind of help you need and if there are any special needs.
